Christie's PlaceStrengthening Systems: Integrated HIV/AIDS Services for Women, Children and Families A pioneer of the family-centered model that is now considered a best practice intervention by social service organizations; Christie’s Place addresses the multiple barriers faced by HIV-positive women in accessing and remaining in care. Family Health CentersPrevention & Education: Teen Outreach Mobile Clinic This program provides outreach, HIV/STI screening, access to medical care and referrals to homeless, runaway and /or at-risk San Diego youth, ages 12 to 24. San Diego Volunteer Lawyer ProgramCare and Treatment: HIV/AIDS Enhanced Legal Services This program is the sole free legal resource in San Diego County for Persons Living with HIV/AIDS, targeting low-income consumers who have difficulty accessing legal services through existing legal clinics. San Ysidro Community Health CenterStrengthening Systems: CASA/Our Place Early intervention centers helping to increase access to care for people living with HIV/AIDS from South Bay and Southeastern San Diego, focusing on these communities’ Latinos and African Americans, with particular emphasis on MSMs. Vista Community ClinicCare and Treatment: VCC Care Services for Positives This program will provide individual counseling for HIV-positive Vista Community Clinic patients for the purpose of improving treatment compliance, reducing risk behavior and improving quality of life. North County Health ServicesPrevention and Education: HIV Outreach, Testing and Counseling Services This program provides HIV outreach, education, testing and counseling services with special emphasis on Latino agricultural workers and women of color living in north San Diego County. Mama's KitchenCare and Treatment: AIDS Nutrition Program This program targets San Diego County residents who are struggling with AIDS-related disabilities, or those who are HIV symptomatic, to the extent that they cannot shop or cook food. San Diego Youth ServicesPrevention and Education: Peer Education Empowerment Program (PEEP) This program provides an effective health education and prevention model designed for high-risk youth. Fraternity HouseAIDS Caregiving & Transportation Programs For the past 22 years, this program has been San Diego County’s only licensed provider of residential care homes for men and women living with HIV/AIDS. San Diego HospiceCare and Treatment: HIV/AIDS Case Management Program This program focuses on addressing the needs of adults and children affected by HIV or AIDS using a team approach to provide case management, including both a registered nurse and MSW case managers. Family Health Centers2012 Meth Special Initiative Grant The CLEAR of Meth Initiative will consist of five, incentivized individual CORE sessions, and up to six group sessions, using the CDC’s Choosing Life!
